Form not rendering correct

This is almost working perfect. But still can not start our ad campaign around this spanish page because
-the form is not translating properly
-it's leaving out whole fields and
-not submitting
-before switching the language it works fine.

Hi,
You will need to change the <form> action attribute to include relative path.
