Joomla 1.7 - Language codes in the middle of the URL

I have a site converted from Joomla 1.5 to 1.7 using jUpgrade.
I have installed gTranslate for 1.7, but I am seeing that the language strings are being inserted in the middle of the URL rather than at the end.
What's the most likely fix? I am thinking that maybe I need to make sure I have the right .htaccess rules defined.
